Responsibilities of this role:

  • Multiple place - based teams delivering a preventative offer to local people and unpaid carers approaching adult social care for the first time.
  • To promote good conversation and enable exploration of appearance of care and support needs to agree together and prescribe meaningful preventative support:
  • Strength Based Conversations to explore personal strengths, strengths in existing networks, build resilience in networks and link people in with local community strengths and supports.
  • Deliver preventative offers: Advice and Information (including signposting), Care Technology, Trusted Assessment of Minor Equipment, One Off Direct Payment and refer to Community Reablement.
  • Following robust exploration of preventative supports, eligible ongoing care and support needs to be explored via SSA Combined Assessment and Support Plan DASCPH Staff Guidance Assessments to enable support planning and proportionate review of any new long-term service.

Requirements of this role:

  • Must be a Qualified Social Worker, registered with Social Work England.
  • Must have a Social Work qualification (Diploma in social work, social work degree or equivalent)
  • Must have minimum of 3 years experience working as a Qualified social worker
