Adult Social Worker, LD & Transitions, LondonRole OverviewPertemps are seeking Social Workers with a minimum of 2 years' experience to join the Adults and Transitions Team. There are two different roles available, both working with individuals with learning disabilities who are Care Act eligible.Learning Disabilities Adults – Backlog Reviews/Assessments
  • Focused on clearing a backlog of Care Act reviews and assessments.
  • Requires a highly efficient and proactive Social Worker who can complete tasks in a timely manner.
  • Strong knowledge of Care Act 2014 and strengths-based approaches essential.
Young Adults (Transitions) – Ages 18-25
  • Working with young adults (18-25) with learning disabilities transitioning to adult services.
  • Key responsibilities include:
    • Care Act Assessments
    • Support planning
    • Reviews
    • Safeguarding and case management
  • Statutory work including risk management and best interest decision-making.
Key Requirements:
  • Qualified Social Worker with 2+ years post-qualification experience.
  • Social Work England (SWE) registration.
  • Strong experience in Care Act Assessments, reviews, and support planning.
  • Knowledge of safeguarding, risk assessment, and mental capacity assessments.
  • Ability to work independently and manage a caseload of 20-25 cases.
