About the Role:

We are seeking experienced and passionate Adult Social Workers to join our dynamic Integrated Independence Team (IIT). The IIT is an innovative partnership between the Homerton University Hospital Foundation Trust and the London Borough of Hackney. We are committed to providing high-quality, person-centred care that empowers individuals to live independent and fulfilling lives within their communities.

Key Responsibilities:

  • Manage a caseload of individuals and carers, conducting needs assessments, developing care plans, and providing ongoing support and guidance.
  • Work collaboratively with individuals, families, carers, and other professionals to ensure coordinated and effective care.
  • Respond swiftly to service user needs, promoting independence and choice through a range of interventions, including home-based support, short-term care, and community-based services.
  • Conduct risk assessments and implement appropriate interventions to minimize and manage risk.
  • Make best interest decisions where necessary and ensure compliance with relevant legislation and professional standards.
  • Provide support, guidance, and direction to colleagues and assist senior staff as required.
  • Maintain accurate and up-to-date records in line with professional and organizational guidelines.
  • Adhere to the standards of proficiency set by the Health and Care Professions Council (HCPC) and participate in ongoing professional development.
