Adult Social Worker

Are you a dedicated Social Worker with a passion for supporting adults to live independent, empowered lives?

This company is seeking an Adult Social Worker to join its forward-thinking team within Adult Social Care. You will play a pivotal role in promoting independence, protecting adults at risk, and delivering person-centred support under the Care Act 2014 framework.

The Role:
  • Conduct strength-based assessments and implement tailored care and support plans.
  • Actively participate in safeguarding enquiries and Best Interest Assessments.
  • Monitor and evaluate services to ensure they meet eligible needs.
  • Work collaboratively with multi-disciplinary teams and external agencies.
  • Chair case conferences and planning meetings, including Safeguarding.

What You'll Need:
  • Degree in Social Work and registration with Social Work England.
  • Experience in community and/or hospital settings, delivering adult care services.
  • Comprehensive knowledge of Adult Social Care legislation (Care Act 2014, MCA 2005).
  • Ability to manage workloads, chair meetings, and make decisions under pressure.
  • Enhanced DBS clearance and the ability to travel as required.
