About the Role:
Join our supportive and dynamic team as an Adult Social Worker in Islington, where you'll play a key role in empowering and supporting adults in need of social care services. Youll assess, plan, and implement care solutions for a diverse range of individuals, ensuring they receive the best care tailored to their needs. Furthermore, one of the roles is working with Young Adults aged 18-25 who have a learning disability and are Care Act eligible. The role will involve undertaking Care Act Assessments, completing support plans, completing reviews, safeguarding (all statutory work) and case management/oversight that is required.

End Date :13/07/2025

Hours Per Week :35.00

Potential to continue or renew contract.

Key Responsibilities:

  • Conduct assessments and develop care plans

  • Advocate for and support vulnerable adults in the community

  • Work collaboratively with multidisciplinary teams and external agencies

  • Provide advice, information, and support to service users and their families

Caseload is 20-25. Flexible working arrangements can be offered but all visits and majority of meetings need to be in person. Need to be in the office around twice a week.
