Job Title: Adult Social Worker
Location: London Borough of Wandsworth
Salary: Up to £32 per hour
About the Role:
We are seeking a highly motivated and experienced Adult Social Worker to join our supportive and friendly team within the London Borough of Wandsworth. In this role, you will be responsible for providing high-quality social work support to adults with a diverse range of needs, including those with physical disabilities, learning disabilities, and mental health conditions.
Key Responsibilities:
- Needs Assessment: Conduct comprehensive assessments, including Care Act Assessments (CAAs) and Strength-Based Assessments (SBAs).
- Support Planning: Develop and implement person-centred support plans, ensuring they meet individual needs and reflect best practice.
- Reviews: Undertake regular reviews of support plans, ensuring their ongoing effectiveness and making necessary adjustments.
- Safeguarding: Identify and respond to safeguarding concerns, ensuring the safety and well-being of vulnerable adults.
- Mental Capacity Act: Apply the Mental Capacity Act 2005 in all aspects of social work practice.
- Duty Work: Participate in the duty rota, responding to urgent referrals and providing initial assessments.
- Case Management: Manage a manageable caseload of no more than 20 cases, allowing for thorough and effective support.
- Liaison: Work closely with other professionals, including healthcare providers, family members, and other agencies, to ensure coordinated and effective support.
- Advocacy: Advocate for the rights and needs of service users, ensuring they have access to the support they require.
Essential Skills and Experience:
- Qualified Social Worker with a relevant professional qualification (e.g., DipSW, CQSW, Social Work Degree)
- Significant experience working with adults with a range of needs within a local authority setting
- Excellent knowledge of the Care Act 2014 and relevant legislation
- Understanding of safeguarding procedures and the Mental Capacity Act 2005
- Strong assessment, planning, and evaluation skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a supportive team
- IT literate with experience of using case management systems
- Committed to a minimum of 2 days per week in the office.
Desirable Skills and Experience:
- Experience of working with specific needs, such as learning disabilities or mental health conditions
- Knowledge of relevant safeguarding procedures
- Experience of working with carers
