Adult Social Worker - South London - Up to £48,000 - Perm -ASYE possible - hybrid (2 days in the office after completing the training)
Our client is looking for a dedicated Qualified Social Worker to join their Adult Services Transition Team. You will conduct Care Act 2014 compliant assessments, develop person-centred support plans, and work collaboratively with NHS partners to secure Continuing Healthcare (CHC) funding for eligible individuals. You'll be responsible for monitoring, reviewing, and updating care plans, advocating for young people's rights, and safeguarding their welfare. Additionally, you will manage a varied caseload and work closely with multidisciplinary teams, including education, healthcare, and community partners.
To be considered, you must hold a Social Work Degree (BA or MA) or equivalent and be registered with Social Work England. Experience in adult social services and working with vulnerable individuals is essential. You must also have a current, valid driving licence and access to a car.
Our client offers a supportive working environment with a comprehensive induction program, regular supervision, and ongoing training. Flexible working arrangements are available, with a minimum of two office-based days each week.
