This South London Authority are looking for a Qualified Social Worker to work within an hospital admissions team. Details of the role are:
Candidates must be willing to work on site in the hospital. THERE IS NO REMOTE WORKING FOR THIS POST.
We are looking for an experienced Social Worker to work on site closely with Health colleagues to facilitate a safe return home for Vulnerable adults who present in the Emergency Department .
You will be required to complete assessments under the Care Act 2014 and Mental Capacity Act 2005. Also arranging care support to enable adults to return home to reduce the risk of social admissions to hospital or temporary placements to manage initial risks until further assessments can take place. You will attend daily board meetings to discuss patient flow and plan the adult’s return home. Once the adult has returned home the case will be followed up by the respective Locality teams.
To apply for this role you must have:
- A relevant Social Work qualification (BA/MA Social Work/DipSW or equivalent)
- Be Social Work Registered
- Be eligible to work within the UK
