Job Title: Adult Social Worker
Location: Royal Borough of Kensington and Chelsea
Salary: £32 per hour
About the Role:
We are seeking a highly motivated and experienced Adult Social Worker to join our Complex Needs Team within the Royal Borough of Kensington and Chelsea. You will be responsible for providing high-quality social work support to adults with complex needs, including those with physical disabilities, learning disabilities, and mental health conditions.
Key Responsibilities:
- Needs Assessments: Conduct comprehensive Care Act assessments, including completing CHC checklists and utilizing Decision Support Tools.
- Support Planning: Develop and implement person-centred support plans, ensuring they meet individual needs and reflect best practice.
- Reviews: Conduct regular reviews of support plans, ensuring their ongoing effectiveness and making necessary adjustments.
- Mental Capacity Act: Complete decision-specific Mental Capacity Act assessments and support individuals and their families in making informed decisions.
- Case Management: Manage a caseload of complex cases, ensuring timely interventions and appropriate service delivery.
- Liaison: Work closely with other professionals, including healthcare providers, family members, and other agencies, to ensure coordinated and effective support.
- Advocacy: Advocate for the rights and needs of service users, ensuring they have access to the support they require.
Essential Skills and Experience:
- Qualified Social Worker with a relevant professional qualification (e.g., DipSW, CQSW, Social Work Degree)
- Significant experience working with adults with complex needs within a local authority setting
- Excellent knowledge of the Care Act 2014 and relevant legislation
- Experience in completing CHC checklists and utilizing Decision Support Tools
- Experience in completing Mental Capacity Act assessments
- Strong assessment, planning, and evaluation skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multi-disciplinary team
- IT literate with experience of using case management systems
Desirable Skills and Experience:
- Experience of working with individuals with specific needs, such as learning disabilities or mental health conditions
- Knowledge of relevant safeguarding procedures
- Experience of working with carers
