What You Will Do

  • Assess Needs – Conduct comprehensive assessments to identify the needs of adults and develop personalised care plans.
  • Coordinate Care – Collaborate with multidisciplinary teams to implement and monitor effective care and support plans.
  • Safeguard Adults – Fulfil safeguarding responsibilities by ensuring vulnerable adults are protected from harm and abuse.
  • Case Management – Attend and contribute to strategy discussions, case conferences, and key decision-making forums.
  • Record Keeping – Maintain accurate, timely records and ensure all documentation complies with legal and regulatory standards.
