The Role:
The role is for an adult transitions team, Undertaking Care Act assessments and associated work with young people transitioning into Adult services from Childrens services.
Essential Requirements:
* Social Work Qualification - Degree
* Experience as a Social Worker working with transitions teams
* Social Work England Registration
