Autism / Learning Disabilities Key Worker North West London - West London NHS Trust Rate: Band 6, £30-£52 depending on experience.6 month contract with view to extend Spencer Clarke Group is seeking an experienced Autism / Learning Disabilities Key Worker in North West London.This role focuses on providing support to a small caseload of children and young people up to the age of 25 that are at risk of admission to, or placed in, specialist mental health inpatient settings.Role Responsibilities:

  • To act as the main point of contact for CYP and their family and strengthen links with relevant professionals.
  • To attend and contribute to community and inpatient Care (Education) and Treatment Reviews
  • Maintain clear, accurate records of work with each young person in line with local procedures
  • Engaging and liaising with stakeholders (including service users) that supports the provision of person-centred care

Essential Requirements:

  • Professional registration ideally within CAMHS remit such as: Social Worker, Nurse, teacher, therapist or significant experience working with children and young people with additional needs.
  • Enhanced DBS check, children and adults
