Care Assessor

Our client is looking for a dedicated and skilled Care Assessor to join their Adult Social Care team within the Learning Disabilities Support Planning Team. This is a rewarding role where you will conduct social care and joint health assessments, ensuring that individuals receive the support they need. You will be responsible for developing comprehensive support plans tailored to the needs of vulnerable adults, including those with complex health conditions, learning disabilities, and mental health challenges.

The role requires the ability to evaluate different care support options, including reablement, supported living, and traditional care services. You will also play a key role in multidisciplinary discussions, liaising with internal and external partner organisations to ensure high-quality care provision. Additionally, you will monitor and review care packages to ensure their continued effectiveness and respond to emergencies where care arrangements may be at risk of breaking down.

Successful candidates should have experience working in social care services, the NHS, or the voluntary sector. A background in assessing needs, creating support plans, and working with vulnerable adults in a professional capacity is essential.
