Care Assessor / Social Work Assistant

Are you an experienced care assessor or social work assistant with a passion for supporting older people and adults with disabilities? On behalf of our client we are looking for a dedicated professional to join our team and help deliver high-quality care to vulnerable adults.

This role involves conducting social care and joint health assessments, developing tailored and comprehensive support plans to address individual needs, and ensuring these plans are monitored and reviewed regularly for effectiveness. You will work closely with multidisciplinary teams and external agencies to ensure the best outcomes for individuals. Safeguarding is a key part of this role, requiring a thorough understanding of local policies and a commitment to protecting those at risk. Creativity and resourcefulness are essential, as you will identify and research both formal and informal care solutions.

The ideal candidate will have experience in social care or related fields such as the NHS or voluntary services, with a strong understanding of the needs of older people and adults with disabilities, including those with learning disabilities and mental health challenges. Excellent communication and planning skills are essential, alongside the ability to produce high-quality assessments, support plans, and reviews. A sound knowledge of social care policies, legislation, and best practices is required, and a willingness to continually develop professionally is highly valued.

While no formal qualifications are necessary at the entry level, working towards or holding NVQ Level 3 or equivalent is desirable for progression. Higher grades may require the achievement of relevant professional qualifications, such as nursing or sensory impairment certifications.
