Job Title: Case Worker

Location: North West London

Rate of Pay: £18 P/H (Umbrella)

Hours per Week: 37.5 Hours

We are currently recruiting for an experienced Case Worker to provide accommodation and pathway to independence services for former rough sleepers within North West London.

Main Duties:

  • Manage a changing caseload, conducting regular key working sessions and creating SMART Support Plans to promote client progression through their pathway.
  • Complete comprehensive needs and risk assessments, using specialist tools to engage clients in personalised support planning and accompanying them to appointments when necessary.
  • Collaborate with internal and external agencies to meet client needs, including arranging case reviews and supporting clients with housing benefit claims and accessing welfare benefits.
  • Support clients in preparing for independence through activities, education, training, and securing work placements, while providing crisis intervention when needed.

The Candidate:

  • Extensive experience working with individuals facing mental/physical health issues, substance misuse, challenging behaviour, and other complex needs, with a strong understanding of relevant housing and related legislation.
  • Proven ability to support clients into supported accommodation, manage risk, and collaborate with service providers to improve client services and outcomes.
  • Skilled in motivating clients toward independence, assisting with budgeting, benefit claims, and rent arrears, while adhering to safeguarding procedures and delivering high-quality risk management.
