Lead Assessor – Adult Social Care - SW London – Hybrid - £40,000

A well regarded charity operating within the social care sector are looking for a Lead Assessor to join them on a 6 month fixed term contract.  Please note – candidates will need to be able to start within 2 weeks notice.

As the interim Lead Assessor you will work collaboratively to conduct thorough assessments including sensory and environmental needs to ensure individual receive the appropriate care and support and are living in homes that support their needs.  Key responsibilities will include:

Performing detailed assessments of sensory and environmental needs, dynamic risk assessments and evaluating suitability of accommodation; making recommendation for suitable support packages.

Monitor and respond to incoming referrals ensuring timely and thorough assessments are delivers

Utilise accessible methods to conduct assessments ensuring individuals and their families can partake

Work closely with social workers and other support staff to gather information and make informed decisions

Create individual, outcome focused support plans

To be suitable for this Assessor (Social Care) role you will have the following relevant experience:

Strong background in adult social care with knowledge of learning disabilities, autism and mental health

Extensive experience in referrals, assessments and placement management with proven expertise in assessing peoples needs

Strong knowledge of the Care Standards and Care Act, safeguarding DOLS/MCA and CQC framework

The salary on offer for this role is up to £40,000 pro rata.
