What You Will Do
- Conduct Mental Health Assessments – Perform comprehensive assessments and deliver effective interventions to support hospital discharges.
- Care Coordination – Work with recently discharged individuals, ensuring smooth transitions back into the community.
- Care Plan Implementation – Develop, implement, and review social care plans tailored to adults with mental health needs.
- Multi-Disciplinary Collaboration – Collaborate with healthcare professionals to deliver integrated, patient-centered care.
- Communication & Advocacy – Conduct face-to-face and virtual meetings, ensuring clear communication and best outcomes for service users.
