As a Social Worker in the Mental Health Team, you will provide crucial support to adults experiencing mental health challenges, working collaboratively to deliver personalised and recovery-focused care. Your role will involve conducting thorough assessments, developing care plans, and coordinating with multidisciplinary teams to ensure the best outcomes. You will take the lead on safeguarding responsibilities, manage complex cases, and offer crisis intervention when necessary.

Responsibilities include:

  • Complete detailed assessments to determine clients' mental health needs.
  • Conduct mental capacity assessments in line with the Mental Capacity Act 2005.
  • Submit Deprivation of Liberty Safeguards (DoLS) applications when necessary, ensuring legal requirements are met and individuals' rights are upheld.
  • Provide immediate intervention and support during emergencies or crises.
  • Collaborate with a multidisciplinary team, including psychiatrists, psychologists, nurses, and other professionals, to ensure coordinated care.
  • Keep accurate and timely records of assessments, interventions, and care plans, ensuring all documentation adheres to organisational policies and legal obligations.



Requirements:

  • Relevant social work qualification
  • 2 experience in adult social work, including assessment and care planning.
