As a Social Worker in the Mental Health Team, you will provide crucial support to adults experiencing mental health challenges. Your responsibilities will include conducting in-depth assessments to identify individual needs, developing personalised care plans, and collaborating with various services to ensure seamless access to appropriate mental health support. You will prioritize person-centered, recovery-focused care while ensuring compliance with statutory duties under the Care Act 2014 and the Mental Health Act 1983.

Responsibilities include:

  • Conduct thorough assessments of clients to identify mental health needs.
  • Carry out mental capacity assessments in accordance with the Mental Capacity Act 2005. Make applications for Deprivation of Liberty Safeguards (DoLS) where appropriate, ensuring legal compliance and protecting individuals' rights
  • Respond to emergencies and crises, providing immediate support and intervention.
  • Work with a multidisciplinary team, including psychiatrists, psychologists, nurses, and other healthcare providers, to coordinate comprehensive care.
  • Maintain accurate, clear, and timely records of assessments, interventions, and care plans, ensuring all documentation is in line with council policies and legal standards

Requirements:

  • Relevant social work qualification
  • 2 experience in adult social work, including assessment and care planning.
