About Our Client: Greenwich Council are seeking a dedicated and experienced Community Learning Disability's Social Worker to join their service. Their organization is committed to delivering personalized, integrated care and support to adults with social care needs, empowering them to maintain their independence and wellbeing. Responsibilities: As a Community Learning Disability's Adults Social Worker, you will be responsible for:

  • Conducting assessments and developing care and support plans that enable individuals to achieve their desired outcomes and maintain their independence.
  • Collaborating with adults, their families, carers, and other professionals to provide creative, person-cantered support that upholds individual rights.
  • Facilitating access to advice, support, and services for adults and families, drawing on your extensive knowledge of social care legislation, policies, and resources.
  • Managing complex cases, making informed decisions, and taking proactive measures to support and protect adults and their families.
  • Safeguarding adults and children at risk by conducting thorough assessments and reviews in line with current legislation and best practice.
  • Continuously developing your knowledge of social care legislation, best practices, and local resources, and contributing to a culture of learning within the organization.
  • Recognizing your professional limitations and seeking advice and expertise as needed to ensure safe and effective practice.
  • Contributing to the development of new initiatives, practice improvements, and policy enhancements within our organization and in collaboration with external agencies.
  • Sharing your expertise and providing guidance and support to colleagues within the council and external organizations.
  • Maintaining accurate and detailed records of your activities, ensuring that all information is recorded in compliance with best practices and local guidelines.

Requirements: To succeed in this role, you should possess the following qualifications and skills:

  • A recognized social work qualification and active registration with the appropriate regulatory body.
  • Proven experience in working with adults with learning disabilities and managing complex cases.
  • Strong knowledge of social care legislation, best practices, and current policies.
  • Excellent communication and interpersonal skills.
  • The ability to work both independently and collaboratively within a multidisciplinary team.
  • Dedication to upholding individual rights and promoting the well-being of adults with learning disabilities.
