Qualified Social Worker, Deputy Team Manager, Hospital Discharge TeamPay rate £40 per hourContract roleVitalis are recruiting for an experienced Social Worker to work as a Deputy Team Manager in a Hospital Discharge Team in Southwark Council.The Hospital Discharge Transfers of Care is at the heart of Southwark’s commitment to providing seamless and effective care transitions from hospital to home or other care settings. Our team is dedicated to managing the complexities of hospital discharges, safeguarding concerns, and care planning with precision and compassion. As a leader during the weekend, you will ensure that services remain robust, responsive, and focused on the needs of our residents even during peak - Operational Leadership: Oversee the day-to-day operations from Friday to Tuesday, ensuring social workers efficiently triage referrals, allocate cases, and authorise assessments, keeping the flow of care moving smoothly. - Guidance and Support: Provide expert guidance on complex discharges, safeguarding issues, and mental capacity assessments, ensuring your team is well-supported and empowered to make informed decisions. - Collaborative Networking: Serve as the lead manager for weekend hospital discharges at Kings and St Thomas Hospital, ensuring that care plans are maintained, and discharges are handled effectively.
