We are looking to recruit a Qualified Social Worker - Adults with disabilities within Harrow Council
Responsibilities include:
- Care Act Assessments
- Support Planning and Reviews
- CHC Assessments
- Asylum/NRPF, Transition, Safeguarding
- Undertaking comprehensive social care assessments and reports, with detailed support plans, as appropriate.
- Knowledge and understanding of Direct Payments using the strength-based approach
- Social Worker should have sound knowledge around the continuing health care process and procedure and the legislation that underpins this, including the Decision Support Tools.
- Social worker will also need to communicate closely and sensitively with family members of service users.
Work arrangements: Flexible arrangements to include Face to face visits/assessments, Work from Home, and office as required.
Experience preferred:
- Post qualifying experience of working in a Qualified Social Worker - Older People position within the UK Social Care industry.
Minimum Requirements:
- SWE Registration
- 1 year post qualifying experience within the UK Social Care industry
- Enhanced DBS/CRB (we can obtain for you)
- Eligibility to work in the UK
