We are looking to recruit a Qualified Social Worker in the Integrated Independence Team within Hackney. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels.

The responsibilities of the Social Worker includes:

  • he Integrated Independence Team (IIT) is an integrated health and social care service provided jointly by the Homerton University Hospital Foundation Trust and the London Borough of Hackney.
  • The service places the user at the heart of the care planning process, and aims to promote independence, deliver outcome focused care, and provide a seamless journey through the care system.
  • The service will have a single point of access and use a multi-disciplinary assessment and care planning framework.
  • The service will respond swiftly to service user need and, where possible, will treat people in their own home or normal place of residence.
  • The service will also provide short term bed based intervention as appropriate.
  • This will be in partnership with individuals, carers, professional staff in other agencies and service providers to establish arrangements and services that meet their assessed needs and promote independence, choice and control.
  • To manage a caseload of individuals and carers who require additional support or guidance in order to identify their needs and who may need close monitoring or frequent reviews and interventions to minimise and manage risk or where best interest decisions may be required. To provide assistance, guidance and direction to colleagues and assist senior colleagues and managers as required.

To ideal candidate will have:

  • Social Work England registration
  • Eligibility to work in the UK
  • Social Work qualified (MA/BSc/DipSW/CQSW/MSc/BA)
