We are looking to recruit a Qualified Social Worker, Older Peoples Team within the Social Care Industry. Our client has a well-structured and organised team that are looking to grow their staffing levels. The role offers flexible working hours for working parents and has a supportive team culture with regular supervision on cases. This is an exciting opportunity to join a fast-paced and supportive team within Merton Council.Main duties include:
- Carrying out assessment's for Older Persons and their carers
- Obtaining multi-disciplinary assessment of need
- Care management
- Designing intensive care packages
- Carrying a caseload of service users
- Make recommendations for short-term care, intermittent care and permanent residential care where appropriate
- Self-directed support / personal budgets
To be successful:
For this Qualified Social Worker, Older Peoples Team you will need to be a Social Worker with a HCPC Registration and educated to Degree Level. You will also need to have the Eligibility to Work in the UK with previous experience working within an Adults' Older Peoples Team.
