We are looking to recruit a Qualified Social Worker in the Transitions Team within Islington. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels.

The responsibilities of the Social Worker includes:
- Working with Young Adults aged 18-25 who have a learning disability and are Care Act eligible. The role will involve undertaking Care Act Assessments, completing support plans, completing reviews, safeguarding (all statutory work) and case management/oversight that is required.
- Caseload is 20-25.
- Flexible working arrangements can be offered but all visits and majority of meetings need to be in person. Need to be in the office around twice a week.
To ideal candidate will have:
- Social Work England registration
- Eligibility to work in the UK
- Social Work qualified (MA/BSc/DPSW/CQSW/MSc/BA)
