We are seeking a Qualified Social Worker for the Hospital Discharge Team at Wandsworth Council.
Candidates should have experience working in a hospital setting and conducting Mental Capacity and Hospital Discharge Assessments.
The role requires Social Work England registration and eligibility to work in the UK, along with a relevant social work qualification.

We are looking to recruit a Qualified Social Worker in the Hospital Discharge Team within Wandsworth Council. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels.

The responsibility of the Social Worker includes:

Experience of working in a hospital setting.
Experience of Mental Capacity Assessments/Hospital Discharge Assessments.
Experience of Safeguarding

To ideal candidate will have:

Social Work England registration
Eligibility to work in the UK
Social Work qualified (MA/BSc/DPSW/CQSW/MSc/BA)
