About the RoleAs a Social Worker in the Adult Social Care team, you will:

Conduct needs-led assessments in line with the Community Care Act.

Develop personalised care plans and coordinate multi-service care packages.

Work collaboratively with service users, carers, and multi-disciplinary teams to ensure seamless service delivery.

Manage complex caseloads, including safeguarding adults and Deprivation of Liberty Safeguards (DoLS).
-
Support the team in achieving performance targets and driving continuous improvement.
-
Ensure all work aligns with Brent Council's policies and procedures.

A qualified Social Worker with current Social Work England registration.

Experience in adult social care.

Strong Front Door or case management skills, with the ability to prioritise effectively.

Excellent communication and teamwork skills to collaborate across services.
