Job Overview:
We are seeking a Qualified Social Worker - Hospital Team to join our Adult Social Care team. The role involves ensuring safe hospital discharges, delivering high-quality social care, and working within a multi-disciplinary team. You will conduct strength-based assessments, support older people and those with physical disabilities, and coordinate safeguarding work while maintaining high practice standards.
Key Responsibilities:

Work with clients, families, and health professionals to ensure smooth transitions from hospital to community care.

Conduct Mental Capacity Assessments and facilitate Best Interest Meetings under the Mental Capacity Act 2005.

Complete assessments, support plans, and reviews in line with the Care Act 2014.

Promote self-assessment tools and encourage independence through reablement services.

Identify risks, create protection plans, and manage cases in a high-paced environment.

Work closely with professionals from statutory and voluntary organizations to achieve the best outcomes.

Identify unpaid carers, offer Carers Assessments, and provide appropriate guidance and signposting.

Stay updated with UK social care laws, best practices, and organizational policies.

Maintain detailed case records and ensure data is entered into the system promptly.

Essential Requirements:

Degree/Master's in Social Work & SWE registration.

2+ years of post-qualification experience.

Strong knowledge of UK social care legislation.

Right to work in the UK.
