Qualifications:

Key Responsibilities:

  • Relationship Management: Maintain positive relationships with patients' families, friends, and external services, including care coordinators, care commissioning groups, and third-sector organizations such as MIND charity.
  • Quality of Care: Ensure that the social care provided to patients is consistently of the highest standard, contributing to their overall well-being and recovery.
  • Collaboration: Work collaboratively with other professional staff to ensure a consistent pattern of care tailored to the individual needs of each patient.
  • Professional Standards: Establish and uphold high standards of professional practice and levels of care within the unit.
