As a Social Worker in the Assessment and Care Team, you will work with individuals across various age groups, assessing their care needs and providing timely support. Your role will involve conducting assessments under the Care Act 2014, developing tailored care plans, and coordinating access to services that promote independence and well-being. You will work closely with service users, their families, and multi-disciplinary teams to ensure that care is delivered effectively, while maintaining compliance with statutory requirements and safeguarding procedures.

Responsibilities include:

  • Perform assessments according to the Mental Capacity Act.
  • Carry out mental capacity assessments in accordance with the Mental Capacity Act 2005.
  • Examine and supervise carer strategies.
  • Responsible for managing a caseload, conducting robust assessments in partnership with service users, their carers and multi-disciplinary partners.
  • Work closely with the Team Manager with a strengths based approach to provide personalised services with a clear focus on quality outcomes for our residents.

Requirements:

  • 2 years experience post qualification
  • Action safeguarding procedures
