Vitalis Social Care is seeking an experienced Reablement Enabler for a position in the London Borough of Islington.
The role offers a pay rate of £22.76 per hour and focuses on delivering personal care and support according to personalized Reablement plans.
Responsibilities include personal care tasks, domestic tasks, and implementing exercise programs developed by therapists to aid rehabilitation.
The Enabler will promote independence and work closely with health and social care professionals in the care planning process.

Reablement Enabler, London Borough of IslingtonPay rate £22.76 per hour Vitalis Social Care are recruiting for an experienced Reablement Enabler to join a team in Islington.Job Purpose & Responsibilities: - The post holder will deliver personal care and support to people in accordance with their personalised support/Reablement plans. This may include:· Personal care tasks· Domestic tasks· Exercise programs, developed by therapists to support people to achieve agreed outcomes and add to their rehabilitation or recovery at home.Care and support are given in an enabling way, and the focus is on enablement and the promotion of independence.
