Senior Practitioner - Localities Front Door Team

We are looking to recruit a Senior Practitioner within the Localities Front Door Team in Haringey.

Responsibilities include:

  • To provide case management, guidance, support and expert advice to social care staff, students, and partner staff as and when required, maintaining the highest level of professional service.
  • Assess and prioritise incoming referrals, from members of the public or other professionals or services: formulate an appropriate response and take action tore solve or refer as appropriate.
  • To provide professional supervision for social care staff including undertaking regular case file audits, and to identify continuing professional development and training needs of the staff working with the Team Manager to meet these needs.
  • Undertake emergency and other assessments of needs, risks and options for adults with the most complex/high risk care needs and/or circumstances, in accordance with legislative requirements, all relevant policies and procedures and agreed performance targets.
  • To provide advice, information and guidance to customers and their carers ensuring that the relevant legislative framework and councils policies are adhered to.
  • To undertake Safeguarding Enquiries under Section 42 of the Care Act 2014 and work as a Safeguarding Adult's Manager when required.
  • Undertake a Act 2014 assessment and support planning for more complex cases when necessary. May be required to hold a limited case load of more complex cases eg legal and safeguarding work.
  • Chair Safeguarding meetings/ planning meetings/ case conferences/ strategy meetings as appropriate to specialised field/ local structures.
  • To assist the Team Manager in leading, setting and implementing a performance management framework and ambitious service performance targets, practice standards and competencies within employee's profession fields of work.
  • To support the Team Manager in managing the Team to deliver high quality customer services to the public relating to adult social care and safeguarding, ensuring all customer service targets are met.
  • To work collaboratively with other managers across Adult Services and the wider health and social care system to maximise independence and improve outcomes for service
  • To refer for relevant assessments as required e.g. Care Act Assessment, Human Rights Assessment, Risk Assessment, Carers Assessment to support individuals with particularly complex needs
  • Responsible for providing expert social care advice and support to a team of social care professionals in the Safeguarding Team.
  • To ensure that all Adult at risk enquiries are completed in a timely way and transferred to other teams when appropriate.
  • To provide operational leadership and support as directed by the Team Manager to ensure the service achieves its Key Performance Indicators.
  • To support the Team Manager in completion of customer complaints and ensure that they are managed and resolved effectively.
  • To support the Team Manager in ensuring that resources are utilised effectively within the multidisciplinary team.
  • To ensure that the team provide clear, accessible and reliable information to all contacts to Adult Social Care to ensure that they are able to make informed decisions.
  • To effectively manage and monitor specific team member's individual performance to ensure customer service targets are met and high service levels are maintained.
  • To identify training needs of the team and provide coaching and development to ensure they have the skills and knowledge required to effectively carryout their role.
  • To ensure team members maintain data quality and integrity and that data is managed in accordance with business rules on retention, duplication and quality.

Minimum Requirements:

  • SWE Registration
  • 5 year post qualifying experience within UK
  • Enhanced DBS/CRB (we can obtain for you)
  • Eligibility to work in the UK
