Key Responsibilities:
  • Conduct thorough assessments to address mental health and social care needs, ensuring timely hospital discharges.
  • Apply your expertise in statutory duties to manage a complex caseload efficiently.
  • Collaborate with carers and professionals to achieve the best outcomes for individuals.
  • Perform social care needs assessments, Human Rights assessments, and mental capacity assessments.
  • Act as a Safeguarding Enquiry Officer, working closely with the Safeguarding Adults Manager to ensure safe outcomes.
  • Advocate for equality and the effective application of health and social care policies.
Candidate Requirements:
  • Excellent communication and assessment skills.
  • A passion for supporting carers and individuals in the mental health sector.
  • Strong knowledge of adult social care and mental health legal frameworks.
  • An Approved Mental Health Professional (AMHP) Qualification is highly desirable.
