We are seeking a highly motivated and experienced Adult Social Worker to join our dedicated team at St Pancras Hospital. This role is crucial in ensuring that Islington residents receive timely and effective discharge planning, enabling them to return home safely and independently.
About the Role:
As an Adult Social Worker, you will be responsible for:
- Assessing the needs of complex patients, including those with neuro-rehabilitation needs (brain injury, stroke, etc.)
- Developing comprehensive discharge plans that promote patient autonomy and well-being
- Collaborating with a multidisciplinary team, including medical staff, therapists, and care providers, to coordinate care and facilitate smooth transitions
- Providing high-quality social work interventions, such as risk assessments, safeguarding checks, and advocacy
- Maintaining accurate and up-to-date records
About You:
To be successful in this role, you will need:
- A relevant social work qualification (e.g., DipSW, CQSW, MSW)
- Significant experience in hospital discharge planning, ideally within an acute setting
- Advanced skills in assessment, care planning, and risk management
- A strong understanding of the needs of people with complex health conditions, particularly neuro-rehabilitation
- Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals
- A commitment to person-centered care and a passion for improving the lives of vulnerable adults
