Transition and Progression to Adulthood Social Worker. Social Workers required for the Transition and Progression to Adulthood Team supporting young people aged 18-25 with a learning disability or care and support needs under the Care Act. This is a project post for 5 months only with the focus of undertaking assessments and progressing support plans. The successful applicant will be expected to attend meetings and visits in person. The applicant will need to attend the office but can also work from home throughout the week- arrangements for this can be agreed on successful interview. 35 hrs per week.
