- Conduct comprehensive Care Act assessments to cater specifically for extra care sheltered accommodation voids.- Perform carer’s assessments and reviews, ensuring all care needs are being met effectively.- Complete scheduled reviews and serve as a point of contact for service users.- Undertake safeguarding enquiries diligently to ensure the safety and well-being of all clients.- Present cases at various panels, including those involving joint funding, and prepare statements, working alongside the legal team for court hearings.- Participate in duty systems, representing the service at GP Multidisciplinary Team meetings.- Liaise with the Care Support team at Leacroft Lodge and other Extra Care schemes, ensuring seamless delivery of services.Why Newham?Newham offers an invigorating and diverse community vibe, making it a fascinating place to work and live.
