The Role:

As a Social Worker in CIS, you will:

  • Conduct Assessments: Assess needs at home and in community settings, developing person-centred care plans.
  • Work Collaboratively: Team up with Occupational Therapists and Independent Living Assessors to deliver integrated care.
  • Safeguard and Support: Ensure service users are protected, and provide support to families and carers.
  • Proactively Plan: Look ahead to prevent future needs and create sustainable support options.

You will document assessments and work with a multidisciplinary team to deliver meaningful impact.

About You:

You will be:

  • A qualified and registered Social Worker with experience in community settings.
  • Experienced in Mental Capacity Assessments and creating care plans.
  • Familiar with assistive technologies and safeguarding procedures.
  • Competent using Mosaic, System1, and MS Office.

You will need an enhanced DBS check and a passion for delivering high-quality care to diverse communities.
