Our client, a Trust in London, is seeking experienced Social Workers to join their Adult Community Mental Health team. In this role, you'll deliver a statutory social work service to a caseload of patients, ensuring care plans are tailored to meet individual needs. You will work closely with other professionals to safeguard service users, assess their needs, and develop effective support plans.
Key responsibilities also include:
- Advising on alternative approaches to tasks
- Collaborating with colleagues to find solutions to challenges
- Adapting living or working environments to meet service user needs
- Developing strategies to help patients achieve their goals
What youll need:
- Degree in Social Work or equivalent qualification
- HCPC registration
- Experience conducting assessments to identify health and social care needs
- Relevant specialist training in the field
