Connect2socialwork are urgently looking for a Social Worker to join their great Reablement Team with a Pay rate up to 38.50 an hour on a locum basis.

Job Description;

To work in a multi-agency context, sharing information, joint assessments and care planning to provide a holistic understanding of need and provision of services.
To take responsibility for promoting and safeguarding the welfare of vulnerable adults in your care and those who you come into contact with.
To carry a caseload of varied range of complexity.
To take on care coordination responsibility for their caseload.
To undertake Mental Capacity Assessments in accordance with the Mental Capacity Act 2005
To ensure that the Care Act 2014 is implemented in all aspects of their role.
To work with service users, carers and other professionals to minimise and manage risk whilst enabling the service user to maintain their chosen lifestyle as far as possible.
To obtain informed consent for all care coordinated cases.
To ensure that their Team Manager is kept informed of any significant events relating to the service.
To provide advice and consultancy to colleagues within the immediate team as well and to a wider range of stakeholders, which may include the NHS, the Independent sector and members of the public as appropriate
To keep records of work undertaken and entering data on the database in accordance with Council Policy. * To attend and contribute to multi- disciplinary and other meetings as required.
To meet contractual agreement by attending regular supervision.

Requirements as a Social Worker include:

SWE Registration
Eligible to work in the UK
Drive and have access to a car
Post-qualified experience
Degree level or equivalent in Social Work
