  • Location: South London
  • Duration: 3-6 months ongoing
  • Schedule: Monday to Friday (3 Days in, 2 days remote)

  • Rate of pay: £27-£31 p/h

 

A Day in the Life of a Social Worker:

As a Band 6 Social Worker, you will play a pivotal role in supporting adults with varying complexities, including neurological conditions and other traumas, within an inpatient rehabilitation setting. Your responsibilities will include:

  • Conducting comprehensive, autonomous assessments of patients' social and environmental needs.
  • Developing and implementing tailored care and support plans to address identified needs.
  • Providing support and intervention for safeguarding concerns and crisis situations.
  • Using evidence-based practice and specialist skills to assess, plan, implement, and evaluate interventions.
  • Collaborating with healthcare professionals, community teams, and external agencies to ensure seamless care coordination and discharge planning.
  • Maintaining accurate and up-to-date case records in line with professional and organizational standards.
  • Taking a lead role in organizing and addressing the health, social, and emotional needs of patients with neurological and other complex conditions.
  • Keeping current with legislative changes, social care practices, and best practices to ensure high-quality service delivery.
Requirements and Qualifications:
  • Current and valid registration with Social Work England.
  • Proven experience in working with individuals affected by complex trauma, neurological conditions, or mental health challenges.
  • Strong communication, advocacy, and interpersonal skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Dedication to providing compassionate, person-centered care.

Working hours and contract duration:

This position will require you to work from Monday to Friday, but hours can be flexible. The contract is for 6 months
