We are seeking a dedicated and compassionate Social Worker to join our supportive, hardworking team within Adult Social Care. You will play a crucial role in helping individuals to lead independent, fulfilling lives by conducting strength-based Care Act assessments, including carer assessments, and developing creative, person-centered care and support plans that meet eligible needs.

Key Responsibilities:

  • Conduct thorough Care Act assessments and reviews, focusing on strengths and setting up imaginative care plans.
  • Complete Mental Capacity Assessments and related risk assessments as a core part of your day-to-day role.
  • Lead or support safeguarding enquiries, demonstrating a deep understanding of safeguarding principles and practices.
  • Carry out assessments using the NHS Continuing Healthcare (CHC) checklist, representing customers and social care in the Decision Support Tool process.
  • Work with diverse client groups, including older adults and younger adults with varying abilities and disabilities, and from all cultural backgrounds.
