Your new role

This role offers an exciting opportunity for registered nurses, dieticians, pharmacists, and podiatrists to specialise in diabetes care and support NHS service in becoming fully multi-disciplinary.Location: St Mary Hospital, Charing Cross Hospital, Hammersmith HospitalHours: 37.5 hours per weekThe working hours are either 8:00 - 16:00 hrs or 8:30 -16:30 hrs.Pay rate: £28 - £31 umbrella.

Key Responsibilities

Provide expert diabetes practitioner advice to ensure high-quality, individualised patient care.Act as an autonomous expert in managing Type 1 & Type 2 diabetes across inpatient and outpatient settings.Assess, plan, implement, and evaluate care programmes for adults with diabetes, including structured education.Collaborate with the multi-professional team to promote excellent standards of diabetes care.Deliver on defined portfolio responsibilities and maintain high standards of care through benchmarking and audit.

Essential Qualifications and Experience

Current registration with NMC (Nurse), HCPC (Dietician or Podiatrist), or GPhC (Pharmacist) with non-medical prescribing or working towards it.Recognised degree and/or postgraduate diploma.Evidence of CPD in diabetes.Experience in medication initiation and insulin therapy adjustment.Experience in a multidisciplinary clinical diabetes setting.Ability to lead training programmes for staff, patients, and students.Relevant clinical experience, including insulin dose adjustment.Ability to supervise and support junior team members.Strong team player with the ability to work autonomously.Knowledge of national and local diabetes guidelines.

Desirable Skills

DAFNE/BDEC training or equivalent.Degree/MSc level module in Diabetes.Management and leadership qualification.Non-medical/independent prescribing course.Basic counselling skills.Experience in conducting audits and/or research.Advanced clinical reasoning.
