What you will do:

  • Assess, develop and plan specialist treatment for service users with complex mental health and physical problems, including visits to inpatient sites and service users' homes.
  • Carry out detailed moving and handling assessments for service users with multiple pathologies and mental health difficulties.
  • Work within a multi-disciplinary team to provide effective and timely assessments, using advanced analytical skills to evaluate and adapt treatment programmes.
  • Facilitate discharge planning, ensuring service users are prepared to maximise their independence.
  • Utilise your expertise to treat conditions such as falls, strokes, fractures, breathing problems, and other health issues to improve service users' quality of life.

To qualify for this rewarding role, you must be HCPC registered and a car driver with a valid licence.
