Outreach Support Worker – Rough Sleepers   LOCATION:           New Forest.SALARY:                  PAYE £14.94 | Umbrella £18.65DURATION:6 Month ContractHOURS:                    37.5 hours per week Note: WE ARE UNABLE TO PROVIDE SPONSORSHIP OR SUPPORT THOSE ON A SKILLED WORKER VISA.Daily Duties  
  • To respond to reports of rough sleeping in locations around the community in order to engage and build relationships with those found to be rough sleeping.
  • Support a caseload of rough sleepers to access benefit, health & welfare services.
  • Support a caseload of rough sleepers to access accommodation in order to end their homelessness.
  • Work alongside Homeless & Housing Advice Officers and the Supported Lettings Worker – Homelessness.
  • Actively engage with rough sleepers with complex needs at their location
  • Support clients to access benefit, welfare and health services
  • Support a small caseload of rough sleepers in the community, who require practical support to access temporary accommodation or private rented accommodation to end their homelessness
  • Work in partnership with Homelessness and Housing Advice Officers and other Support Workers to enable rough sleepers to follow a pathway toward an accommodation placement
  • Accept referrals from the Service Manager – Housing Options and Homelessness and Housing Advice Officers
  • Ensure that clients are encouraged to work towards an agreed support plan, where appropriate, to help them meet their individual needs and goals
  • Accurately record and update support plans, meeting notes, service user records, reports and other relevant documentation and systems as required
  • Ensure service users are fully aware of their rights and responsibilities
  • Advocate on behalf of clients where appropriate
  • Promote and facilitate client participation across the service and other relevant services required by the client
  • Ensure clients have links to community and voluntary group supports
  • Promote good working relations and communications with stakeholders including colleagues
  • Work with the client’s Homelessness and Housing Advice Officer in the planning, organising and reviewing of the client’s support
  • Liaise and establish good working relationships with all relevant external agencies to provide the best possible service for our clients
  • Ensure all stats/information requested is supplied accurately and to the specified deadline
  • Support other complex needs clients in Temporary Accommodation to sustain their accommodation
  • Attend training, and team/development meetings as appropriate
  • Assist with other duties as required by the Service Manager – Housing Options
  • Provide cover for the Supported Lettings Worker – Homelessness as required.
 Essential requirements
  • DBS within the last 12 months (must have original copy)
  • Provided support to those the Homeless and substance misuse.
  • Driver with own car
