Job Overview:We are seeking a dedicated Band 6 Occupational Therapist to join the Macclesfield District Hospital's Community Rapid Response Team. This role focuses on delivering immediate care to patients in the community, facilitating timely interventions to avoid hospital admissions and support early discharge. You will play a key role in enhancing patient recovery and promoting independence in a community setting.

Key Responsibilities:

  • Provide rapid and effective occupational therapy assessments and interventions for patients in the community.
  • Work within a multidisciplinary team to deliver responsive care for patients in need of urgent occupational therapy.
  • Support patients in their recovery by promoting independence and preventing unnecessary hospital admissions.
  • Contribute to service development and enhance patient care through continuous improvement initiatives.

Key Requirements:

  • Degree in Occupational Therapy and HCPC registration.
  • Previous experience in acute or community care, particularly in rapid response or urgent care settings.
  • Strong communication and problem-solving skills.
  • Ability to manage a fast-paced caseload and work under pressure.
