Job Overview:Become a vital member of the Community Rapid Response Team, delivering urgent, patient-centered occupational therapy services. This role focuses on swift assessments and interventions to prevent hospital admissions and support timely discharges.
Key Responsibilities:
- Conduct urgent assessments for patients in community settings.
- Develop and implement individualized treatment plans to promote independence.
- Collaborate with the multidisciplinary team to coordinate care effectively.
- Support patients with complex needs to avoid hospital admissions.
Key Requirements:
- HCPC registration as an Occupational Therapist.
- Minimum of 6 months NHS experience in community or rapid response settings.
- Strong clinical decision-making and communication skills.
- Ability to work independently in a fast-paced environment.
