OverviewWe are seeking a dedicated Community Enabler to join our team in providing essential support and care services to individuals in need.

The role is to support those that have recently been discharged from Hospital, to provide them with the care they need in order to regain their normal day to day routine, following their recent stay in Hospital.

They are assigned to this NHS community team, for a period of 5-6 weeks, with the goal of gaining a positive outcome after this period of support.

Outline of basic duties -- Assist with medication administration following prescribed guidelines-Following any guidance from the Physio team- Ensure the safety and comfort of individuals under your care

-Assisting with personal care as and when required- Collaborate with healthcare professionals to deliver high-quality care services

-Visiting indivduals within their own home, carrying out duties relating to their care plan that has been organised and recommended by Healthcare professionals

Requirements- Previous experience in care, nursing, or a related field is advantageous HOWEVER it is NOT NECESSARY.- Excellent communication and interpersonal skills

- A genuine passion for care

-The ability to use your initiative

-Someone who has an interest in helping others and wanting to do the best they can for them

-FULL TRAINING AND SHADOWING is offered in this role, so you will feel equipped and confident to carrying out such a rewarding role.

Hours of work (paid for full hours + Mileage)

7am - 1pm AND / OR 4pm - 10pm

Monday through to Sunday.

Ability to choose the shifts you CAN work.

Enhanced weekend rate
