What You’ll Be Doing:Join our supportive community stroke and neuro rehab team and help improve the quality of life for patients recovering from neurological conditions. In this role, you will:

  • Perform comprehensive functional assessments

  • Create and implement personalised rehabilitation plans

  • Deliver evidence-based interventions in a community setting

  • Collaborate closely with multidisciplinary teams (PTs, SLTs, nurses, etc.)

  • Maintain accurate and timely documentation

Requirements & Qualifications:To be successful in this role, you’ll need:

  • Current and valid HCPC registration

  • Experience working with stroke and/or neurorehabilitation patients

  • Strong communication and interpersonal skills

  • Ability to work autonomously and as part of a wider team

  • Passion for delivering high-quality, patient-centred care

Working Hours & Contract Duration:This is a flexible role offering full-time or part-time hours across Monday to Friday. The initial contract is ongoing, with a strong potential for extension based on service needs and performance.
