- Provide a professional, multi-agency social work service to young people leaving care.
- Offer direct advice and support to care leavers in areas such as accommodation, income, health, education, and training.
- Maintain regular contact with care leavers and assist in their development of practical living skills.
- Advocate on behalf of young people with agencies and support them in building negotiation skills.
- Assist young people in maintaining family relationships and developing social connections.
- Regularly review and update Pathway Plans for young people.
- Keep accurate records and reports in line with policies and procedures.
- Support team development by sharing knowledge and participating in team activities.
- Work flexibly, including potential evening and weekend work when required.
- Experience working with young people in a support role.
- Knowledge of The Children (Leaving Care) Act 2000 and The Children Act 1989.
- Understanding of safeguarding, equality, and diversity issues.
- Strong communication skills (verbal and written) and ability to maintain accurate case files.
- Ability to work both independently and as part of a team.
- Full UK driving licence and access to a vehicle for work purposes (please do not apply if you do not drive).
- Enhanced DBS required.
- NVQ Level 3 in Childcare/Social Care or equivalent qualification.
- Experience working in a multi-agency setting.
- Flexibility to work out-of-hours (evenings and weekends if required).
- Willingness to travel outside the Trafford area when necessary.
