Job OverviewWe are seeking an experienced Band 6 Occupational Therapist to join the team at Manchester University NHS Foundation Trust. This role focuses on providing specialist assessment and discharge planning support in an acute medicine environment. It is an excellent opportunity to make a significant impact on patients' lives while working in a collaborative and dynamic team.
Key Responsibilities
- Conduct comprehensive assessments to determine patients' needs in the acute medicine setting.
- Plan and implement effective discharge strategies with patients, families, and multidisciplinary teams.
- Provide expert advice on rehabilitation and ongoing care plans for smooth transitions.
- Maintain accurate and up-to-date documentation in compliance with Trust standards.
- Participate in team meetings, case reviews, and service improvement initiatives.
Requirements
- HCPC registration as an Occupational Therapist.
- Minimum of 6 months NHS experience.
- Proven expertise in acute medicine, with a focus on assessment and discharge planning.
- Strong clinical reasoning and decision-making skills.
- Excellent communication and teamwork abilities.
- Dedication to delivering patient-centered, high-quality care.
